WEBINAR DETAILS:  It is predicted that one in five patients could face a live discharge – and possibly even more for those with a noncancer diagnosis or in a for-profit hospice. Hospice providers continue to be faced with eligibility and length-of-stay dilemmas. Hospice services are promoted, but then they may be discontinued due to ineligibility or other reasons. In addition, hospice providers are measured closely for quality through claims data for burdensome transitions related to live discharges.

The hospice live discharge process can be complex. This webinar will review that process, with an emphasis on ongoing eligibility assessment, discharge planning, the team members that should be involved, and the timelines needed to minimize a burdensome transition. You’ll learn a mechanism to support the care continuum even after your services are discontinued to help your agency monitor and keep track of your live discharges for appropriateness and minimal care disruption.

MEET THE PRESENTER:  Kathy Ahearn, RN, BSN, PHN, ALECC, Inc.

Kathy Ahearn has bachelor’s degrees in nursing and social work and is the CEO and founder of ALECC, Inc., which provides consulting on the post-acute care experience, ranging from clinical staff to executive leadership. She has a unique ability to identify agency challenges and opportunities and quickly provide education, support, and mentoring to organizations. Kathy began healthcare work due to a disabled parent and later a child living with chronic disease. She has experienced healthcare professionally and personally, dedicating her life to it and to working as a change agent for the betterment of all served.

Kathy helped develop pain scales to standardize assessment with the American Pain Society and the Joint Commission. She expanded into medical devices as a Managed Care Director, developing one of the first real-time cloud-based disease-specific software programs to assist patients, clinicians, and managed care organizations to improve outcomes and collect data.
